Registration time as follows:
LAST NAME BEGINNING WITH A - M 9:30 A.M. - 12:00 NOON
N - Z 1:00 P. M - 3:00 P.M.
(Parents or Guardian must arrive by 2:30 PM to allow enough time to fill out paperwork)
PARENTS MUST BRING THE FOLLOWING:
CHILD’S BIRTH CERTIFICATE
RECORD OF IMMUNIZATIONS AND OTHER PERTINENT HEALTH DATA
TWO PROOFS OF RESIDENCY (utility bill, driver’s license, auto registration, check stubs, court order documents, etc.)
THE CHILD BEING ENROLLED, IF POSSIBLE
IMMUNIZATIONS THAT MUST BE COMPLETED BEFORE THE FIRST DAY OF SCHOOL ARE:
3 DOSES -HEPATITIS B VACCINE
4 DOSES - DIPHTHERIA, PERTUSSIS, TETANUS (DPT) (DT)
(fourth dose after age 4)
4 DOSES - POLIO VACCINE
2 DOSES - MEASLES, MUMPS, RUBELLA (COMBINED VACCINE)
(after age 1)
2 DOSES - VARICELLA (chicken pox) immunity, either from vaccination, history of disease
or laboratory testing. (Kdg .or 1st grade)
KINDERGARTEN REGISTRATION SET
Mr. David McDonald, Interim Superintendent of Schools for the Connellsville Area School District, announces Kindergarten Registration will be the week of March 19-23, 2018. To avoid long lines, and for your convenience, we encourage children with the last names beginning with A to M to register from 9:30 a.m. to 12:00 noon. Last names beginning with N to Z will register from 1:00 p.m. to 3:00 p.m. (Parents or Guardian must arrive by 2:30 PM to allow enough time to fill out paperwork)
To be registered, children must be five years of age before the first day of school.
Immunizations need not be completed prior to registration, but completion is necessary to start school. All children, by state law, must have the following:
Three doses - Hepatitis B Vaccine
Four doses - Diphtheria, Pertussis, Tetanus (DPT) (DT) (fourth dose after age 4)
Four doses - Polio Vaccine
Two doses - Measles, Mumps, Rubella (combined vaccine) (after age 1)
Two doses - Varicella (chicken pox) immunity, either from vaccination,
history of disease or laboratory testing. (Kdg. or 1st grade)
At registration, the parent must present the child’s birth certificate, record of immunizations, pertinent health information such as childhood diseases or any other significant health history, two proofs of residency (utility bills, driver’s license, auto registration, check stubs, court ordered documents, etc.)
Parents are encouraged to bring their child to registration.
Registration will be conducted in elementary schools according to the following schedule:

Thursday, March 22, 2018 Springfield Clifford N. Pritts Elementary
Friday, March 23, 2018 Make-Up Day at the Connellsville Area Senior High Registrar’s Office
*Registration includes free vision, hearing, and speech screening for each student.
